At Serenity Service Coordination, our mission is to empower individuals with disabilities and older adults to live with dignity, independence, and purpose in the communities they call home. Through compassionate, person-centered service coordination, we walk alongside each participant in the NHTD Waiver Program — listening first, advocating always, and ensuring that every voice is heard and every need is met. We serve as dedicated advocates, guiding patients and families through the complexities of the healthcare and home care systems. This includes assistance with insurance matters, access to community resources, and education on patient rights.
Continuous Monitoring and Quality Improvement
We actively monitor patient progress and gather ongoing feedback to ensure care plans remain appropriate and effective. This allows us to make timely adjustments and continuously improve the quality of care delivered.
